Privacy Notice

Last updated: August 2026

Buloke Health respects your privacy and is committed to protecting your personal information. This notice explains what information we collect, why we collect it, how we use and protect it, and your data protection rights.

Who we are

Buloke Health is a community physiotherapy service operated by Levi Berruezo. For data protection purposes, Levi Berruezo is the data controller for personal information processed by Buloke Health.

Email: bulokehealth@gmail.com

Information we may collect

Depending on how you interact with Buloke Health, we may collect your name and contact details, preferred method of contact, information provided when making an enquiry or arranging an appointment, and information needed to provide physiotherapy care.

If you become a patient, this may include relevant health and medical history, assessment findings, treatment and rehabilitation plans, clinical notes, appointment and communication records, and payment or invoicing information where applicable.

Health information is special category personal data under UK data protection law and requires additional protection.

Why we use your information

We may use your information to respond to enquiries, arrange and manage appointments, assess your physiotherapy needs, provide safe and appropriate physiotherapy care, maintain clinical records, communicate with you about your care, manage administration and payments, comply with professional or legal requirements, and establish, exercise or defend legal claims where necessary.

We aim to collect only information that is reasonably necessary for these purposes.

Our lawful basis

Depending on the circumstances, we may process ordinary personal information because it is necessary to take steps at your request before entering into a contract, to provide services under a contract, to comply with a legal obligation, or because we have a legitimate interest in operating and administering Buloke Health.

Where health information is processed for physiotherapy assessment, treatment or healthcare, we rely on the applicable provisions of UK data protection law permitting the processing of special category information for the provision of health or social care or treatment.

Website enquiries

When you submit the contact form on our website, the information you provide is used to respond to your enquiry and discuss the most appropriate next step.

Please do not include detailed or highly sensitive medical information in the website enquiry form. Relevant clinical information can be obtained through an appropriate process if you proceed with physiotherapy care.

Website form submissions are processed using our website hosting and form-processing provider, Netlify.

Sharing your information

We do not sell your personal information. Information may be shared where necessary with professionals or organisations involved in your care, service providers that help us operate Buloke Health, or where disclosure is required by law. Where appropriate, information sharing will be discussed with you.

How we protect your information

We take reasonable technical and organisational measures to protect personal information against unauthorised access, loss, alteration or disclosure. Particular care is taken with clinical and health information because of its sensitive nature.

How long we keep your information

We keep personal information only for as long as reasonably necessary for the purpose for which it was collected and to meet applicable legal, regulatory, professional and insurance requirements. Different information may therefore be retained for different periods, and clinical records may need to be retained for longer than general website enquiries.

Information is securely deleted or anonymised when it is no longer required.

Your data protection rights

Depending on the circumstances, you may have rights to request access to your personal information, ask us to correct inaccurate information, request deletion in certain circumstances, ask us to restrict its use, object to certain processing, or request transfer of certain information.

Not every right applies in every circumstance, particularly where information must be retained for clinical, legal or regulatory reasons. To ask a question or exercise your rights, contact bulokehealth@gmail.com.

Complaints

If you have concerns about how Buloke Health handles your personal information, please contact us first so we have an opportunity to address them. You also have the right to complain to the Information Commissioner's Office (ICO), the UK's data protection regulator.
